Helena Opoku, 67, had been identified to Redbridge Council as vulnerable by hospital staff three months earlier. But social services did not assess the risk that she posed to herself or her home. Mrs Opoku died as she used charcoal braziers for cooking and to heat her poorly ventilated home.
Giving a narrative conclusion, Graeme Irvine, senior coroner for East London, said:
“The incident is a concern. There is a risk that future deaths could occur unless action is taken”.
He told council staff in a report: “I believe you have the power to take such action.”
The social care team has until December 8 to respond with details of a plan to prevent similar deaths.
