A 4-year-old Philadelphia boy was killed by a shard of glass when a picture frame fell at his home, authorities said.

The boy, who police identified as Adrian Ortega, was playing upstairs at his Northern Philadelphia home Wednesday night with his sister when the glass picture frame fell from the wall, news station WPVI reported.

The incident sent a piece of glass flying and impaled the boy in the stomach, officials said.

“He received a puncture or cut to his abdomen and that’s when family members transported him to St. Christopher’s Hospital,” Philadelphia Police Chief Inspector Scott Small told WPVI.

The tot was later pronounced dead at the hospital, officials said.

Authorities said they are investigating the circumstances surrounding the boy’s death. It’s unclear whether the boy’s mother, Amanda Velez, was present during the incident, according to WPVI.

Neighbors said they were in a state of shock following the fatal accident.

“It’s just a shame because they are so well loved, so well loved that it’s a complete shock,” Neighbor Edward Rodriguez told WPVI. “When it happened, it just went up and down the neighborhood like lightning.”
